  • Date/time of problem occurrence
  • Factor(s) that trigger the problem (example: SC672-11 occurred 3 minutes after tuning ON the main power switch.)
  • Occurrence frequency (example: one out of ten times when turning ON the main power switch)
  • Parts replaced
  • Date/time when parts were replaced
